SDA provides staffing for every team member in the dental office from Dentists, Hygienists,
and Dental Assistants, to Business Office Staff. We provide coverage for dental offices whether it
is for one day or for an extended leave of absence. Our goal is to provide a professional referral
who has knowledge of your computer operating system, compassion, and the ability to effectively
accommodate your patients. Only the best professional referrals are placed in your available
positions or temporary assignments.

If a Professional Referral leaves your employment within 90 days, we will refund a percentage

of our fees, or send a FREE Professional Referral in his or her place. Synergy Dental Associates’
objective is to build lasting relationships. We strive to satisfy our Clients and we encourage feedback,
positive or negative, on every referral we place.

When Synergy Dental Associates provides a Professional Referral to your office for a maternity

leave or an extended leave of absence, we make every effort to have a maximum of one or two
referrals in his or her place. We understand how important continuity is in the dental office. If for
any reason you are not satisfied with the referral we send, we will unconditionally replace that
person with a referral qualified to suit your needs.

When a request for a "fill-in" or daily referral is made, we make every effort to send the most
qualified individual to your office. In some cases, personal character traits will supersede skill level.
When that is the case, we will notify you. SDA makes a conscious effort to match your individual
needs and personality of your office with the skills and personality of our Professional Referrals.
